The Audiology Department at “Mishkan HaReiyah, the Optical Center Foundation” was founded five years ago in Jerusalem. Its goal is to provide advanced digital hearing aids at subsidized prices to those with impaired hearing.
Since its establishment, the Foundation has provided some 12000 hearing aids to all population sectors in order to achieve its goal of improving the quality of life for the hearing impaired and to make their communication with those around them easier.
The Foundation subsidizes more than 90% of the hearing aid’s cost.
Criteria for children and adolescents between 6 – 20 (does not include married children).

CHILDREN FROM FAMILIES RECEIVING INCOME
Required documents:
- Details of all bank account transactions for the past three months for both parents
- In the event that it is not clear from bank account transactions, then confirmation should be provided by the National Insurance Institute regarding Income supplements/assurance.
CHILDREN OF FAMILIES KNOWN TO THE WELFARE AUTHORITIES
Required documents:
- Up to date confirmation from welfare authorities with entitlement levels.
- If the clients brings a letter from a social letter, they should be referred to the manager.
LOW PARENTAL INCOME
Low parental income – up to an average of 2600 NIS per person (The calculation includes the parents and unmarried children)
Required documents:
- Details of all bank account transactions for the past three months for both parents.
- Wage slips for both parents for the past three consecutive months.
- If one of the parents is self-employed they must provide confirmation from an income tax assessor with details of yearly income.
- In the event that one of the parents is a self-employed worker, a tax assessment must be provided.
- If one of the parents is unemployed, a document must be provided from the National Insurance Institute that confirms that they are unemployed. The document must include details of earnings
- for the last three months.
- Unemployment benefits (Earnings calculations must include the rate of unemployment benefits).
- If one of the parents is unemployed and does not receive unemployment benefits, a document confirming this from the National Insurance Institute must be provided (Confirmation of unemployment status).
- If the father is a Yeshiva student and unemployed an official document must be provided from the Yeshiva confirming this with details of the grant amount.
- If the mother is on maternity leave, wage slips from the last three consecutive months before the maternity leave plus details of the last three months maternity payments from the National Insurance Institute must be provided.
Entitlement is once every three years

NON-WORKING STUDENTS
Required documents:
- Valid student card plus up to date confirmation from the National Insurance Institute of “non-working” status.
THOSE KNOWN TO THE WELFARE AUTHORITIES WITH A LOW INCOME
Required documents:
- Document with entitlement status and level or a letter from a social worker declaring that there exist financial difficulties that justify the grant.
PERSONNES RECEVANT UNE SUBVENTION OU UN COMPLEMENT DE REVENU DU BITUAH LEUMI (``AVTAHAT/ASHLAMAT AHNASSA``)
Documents requis:
- Relevé bancaire du dernier mois ou figure le montant du bitouah leumi
- Justificatif officiel de la subvention du bitouah leumi.
THOSE WITH AN INCOME AS DETAILED BELOW:
Single person: monthly income of less than 4300 NIS
Couple: Monthly income of less 8600 NIS.
Couple with children: monthly income of less than 3000 NIS per person (not including married children up to the age of 20)
Required documents:
- Details of bank account transactions for the last three consecutive months and the applicant’s wage slips for the past three months (if a single person) or for both applicants (If a couple or a couple with children).
- If one of the parents is self-employed they must provide confirmation from an income tax assessor with details of yearly income.

- Certificate/Confirmation letter from the Ministry of Defense
VISUALLY IMPAIRED/BLIND
Required documents:
- Official certification of blindness/visual impairment
The entitlement is for once every two years

HOLOCAUST SURVIVORS
(Entitlement is for the survivor only from wherever in Israel).
Required documents:
- Letter from an official, recognized body
VICTIMS OF TERROR ATTACKS
(The entitlement is only for the victim) Required documents:
- Confirmation letter from the National Insurance Institute or the Ministry of Defense.
THOSE WITH A LOW INCOME
Those with a low income of up to 4000 NIS per month for a single person or 7000 NIS per month for a couple, including pensions and other allowances:
Required documents:
- Bank statements with all transactions for the past three months for each person. *If the account is a joint account please make sure that pensions and allowances for each person are included in the statements.
THOSE KNOWN TO THE WELFARE AUTHORITIES
Required documents:
- A document showing the level of entitlement or, alternatively, a letter from a social worker showing that that there are financial difficulties that warrant the assistance.
ONLY CONSCRIPTED SOLDIERS
Required documents:
- Conscripted Soldier Service Card
- ID card
The Foundation’s management is entitled to change or cancel the criteria at any time and at its sole discretion.
